The Notice of Privacy Practices outlines my office’s privacy policy. A paper copy is displayed in office and you are welcome to obtain a paper copy at any time. The notice is a very long document. I have a attached a shorter version at the beginning of the link that may be preferable to read. The second document listed is a consent form that must be signed before services can begin.
Prior to your first appointment, I will mail you a pre-appointment package. It will include an appointment confirmation, the forms mentioned above, as well as additional documents. You can fill out the paperwork at home or during your first visit.
